Privacy Information for Youth
Health Initiatives for Youth Hamilton believes . . .
. . . you have the legal and ethical right to information about your care and treatment
. . . we have to make every effort to provide the highest level of security and confidentiality with the information that you trust us with
. . . we should collect only the information that we need to provide you with the best care
. . . we can only give out information about you when it is necessary for your care and only when you give us permission to do so, except when we have to obey the laws that affect our services
. . . the information that we have about you is your information, you have the right to look at your chart and have copies of your information. We may have to charge a small fee to make the copies for you
. . . you should have any and all questions that you have about your information answered promptly and correctly
. . . you have the right to ask for information to be corrected if you believe it is wrong
If you are coming in for service you should know:
- You will be given an Informed Consent to Treatment, a form which will give you a detailed explanation about:
- the collection of your information
- your consent for treatment
- the organization’s legal responsibilities
- We have to ask sensitive questions to provide you with the best care. You have the right to ask why we need to know the information and have staff answer your questions.
- You will receive information about all your treatment choices. This will include the expected benefits, risks and possible side effects and what would happen if you did not have the treatment.
- You have the right to say you want to stop receiving treatment at any time. NO means NO!
- Many times while receiving services or medical care you will not be asked to give written consent for every treatment. Usually the staff will talk to you about what will happen next.
- If you feel that the information in your file is incorrect, that you should ask for it be corrected.
- Should your questions or concerns about the information being collected or the way it is collected not be answered to your satisfaction, we have a process to help you with your concern. Our agency has a Chief Privacy Officer who is in charge of the information collected. (see below)
- For us to receive funding to provide our services we are required to release statistical information. This form of information is numbers or percentages such as the number of females or males we served.
Health Initiatives for Youth Hamilton is a non-profit charitable health service organization legally incorporated to operate in Hamilton, Ontario. As such we must obey the following laws:
- Personal Information Protection and Electronic Documents Act S.C. 2000; Personal Health Information Act 2004 about the collection, storage and use of private information
- Child and Family Services Act (1990) about the reporting of suspected child abuse
- Health Protection and Promotion Act (2001) about the reporting of infectious diseases such as gonorrhoea, and syphilis, and sexually transmitted diseases such as chlamydia, and HIV to the Medical Officer of Health
- Mental Health Act (1990) about the reporting of suspected harm to self or others

Health Initiatives for Youth of Hamilton does not sell nor will it release any information related to its donors without their expressed written authorization prior to the release.
